To open a new account with us, please follow these instructions:
We are required by each state to maintain up-to-date information on our customers’ Resale Certificates.
To help us remain in compliance, please provide the necessary documentation as outlined.
- Please complete and send back the enclosed Resale Certificate form.
- A copy of your State Resale (or Sales Tax) License is required.
Please ensure that the following fields are completed and correct when returning the form:
- Your business name as it appears on your State Resale (or Sales Tax) License must match the name used on the Resale Certificate form.
- If you operate as a DBA, then this will be your business name.
- Please also include your business title. Both the description of your business and the description of your product must be included in the required fields.
- The form must be hand signed (or Adobe digital signature) and dated.
⚠️If you drop ship to California, please include your resale for California as well.
